By reconstructing the pre-blast geometry based on construction photos and the blaster’s reports along with the post blast geology, the damage caused by a construction blast was determined to have been avoidable. The primary factors contributing to the incident were the weak zones identifed in the initial geologic investigations that were not accounted for in the blast layout and initiation sequence.
